Ceiling extractor fan fitting but previous leaking issues

Hi there,
Basically need a new extractor fan fitted and whilst fitting is something I could do, the last one leaked through a lot and packed in after a month or two. Assuming it's water coming back down/condensation so was hoping someone could have a look and fit with proper ducting or a condensation trap if required.
The exit is directly vertical to the roof about 2 meters above which could also be part of the problem.
