Depending on your flooring cover it may be due to age or wear, also it could be due to constantly being wet and damp

Absolutely this is essential, scrim and skimming the ceiling must be done first
Pva won't stick
Clean the surface, scratch the paint gently giving 2coat of stronger pva given some time in between the coats

There are number of outlets that provide good quality bathtub like victoria bathroom, depending on how much you really want to spend
