Answers:

If the existing wall paper is firm on the wall free of dust n bubbles ect yes you paper over it.
The plastered wall is one call a (hydroscopic wall) it will suxk water ,so it must be left to dry 100% same applies to the primer and not to for get should be clean and free of dust.
Not all houses will be the same some will need more than others but it must done correctly in some cases the paint has to be peeled or striped off. i would you sand paper garde i.e 100 then 120 to smoothen the wood work and it depends on the age and type of wood.
Exterior wood requires more attention due to the variables of weather, it's important to prep the wood, preserve the wood and protect , it would help to know the type of wood .it can be then decorated to the clients choice with suitable paint there are many different products on the market for this. preservers like carbinol wood preservers good or barrettine premier wood preserve.
In my experience i have used different rollers but it depends on your hand,i.e works very well for me.but there are special rollers for ceiling. applying the paint would be a back and for motion.
