Answers:

Macerator
The macerator is pumping the waste water from the macerator to the soil pipe if it’s pumped up to the loft and then running horizontal to the soil pipe as long as you have a fall on the waste pipe it won’t make any difference if it’s 40mm or 110mm
Fitting new radiator pipe work.
You don’t have to have the heating on to check for leaks i’d personally pressure the heating system to 2 bar and that should be sufficient to test for leaks
