Cost will vary depending on what sort of driveway you would like. full block paved, tarmac, or gravel. the cost between tarmac and paving is very similar, gravel is the cheapest option. only council approval you would need is if you wanted to drop the kerb.
No this is not ok. your builder should have dropped the levels lower and fitted aco drains across the front of your house to take water away. you should have at least 6” between external ground level and your dpc.
60-80mm is fine, obviously it will be a lot cheaper to bring up with type 1 first rather than just use mix
