I personally would scrape it back or i would sand it back
This really depends on the size of the holes every job is different and the placement as well it might be easier if the holes are grouped together to cut out a bigger hole in a neat square shape and patch with a piece of board
No but if the brick work is damaged then it is best to use a skin like rendergrip this helps seal the surface and make a nice coat to apply render to also it's best not to render in cold weather as it takes a while to dry
