it is not always the flooring that makes the noise

Yes but the joist would need to be bolted together with staggered bolts
No but as part of a job would be willing to
As long as the surface between the taps is flat yes you can but you need to make sure the gap between the holes match the tap
You will not buy that size lining off the shelf it would need to be cut and routered out of two lining packs
