the position of your flush button doesn't really matter weather top or side, what maters is the amount of water your flush discharges per flush and that can be adjusted on the flush unit.
i will recommend put flush botton in front panel so you can make use of the top panel for storage.

No i would install a twin shower pumps for both hot and cold water so that the flow rate is the same regulated by the pump.

unfortunately if your service pipe is confirmed to be leaking after a leak detection you have to dig from your meter to the house even if it passes through tarmac, you need to get the necessary permits to do that as well but your service pipe must be minimum 750mm to 1300mm deep.

If your 43mm outer pvc diameter pipe is in a position that's concealed use a rubber ring
