Most of the time its the lintels that are wet somewhere or common fault is that there is not enough air flow nearby to clear the moisture or that there is actually a leak from above.

This is a tricky one because its the access and working platforms that have to be secure to work on and off throughout the works ,this will increase the costing a lot.

This is a no ,the larger lid has to be on the top otherwise lid will drop into inspection chamber.
Yes all paving has to have sand infill > its the sand that when compacted vibrated within the gaps that hold all the paving together like a fixed carpet that once driven on is a total m2 drive that takes the full load of car weight and spreads it out throughout the total drive area complete.
