Answers:

Builder - notifying start date
Most builders like myself would be finishing off a job via quoting one. so id normally give month notice to get stuff in place and ordered .
Convert a carport
Without looking at the job be hard to quote really . its a simply enough job to complete .
Beginning your extension.
I would recommend trying to complete the shell but i think would get away with concreting the footing building footing and pouring the floor .
Patio Mortar Bed Thickness Limit
That is plenty . the mortar bed is fine also
