A leak in the heating system is usually the cause. the combi boilers usually function with a 0.7 to 1.7 bars pressure and if it goes down then there should be a leak in the system.
Vent pipe needs to go above the last window by 80 cm and be connected between 1.8 m from the toilet.
Is fine as long you did not fill more than 2.5 bars and you shut the valves off, or at least one.
Get a reduction. also you could modify the connection and fit a threaded connection with a rubber seal to fit on a bare pipe.
