There will usually be a cartridge locking nut under where the cartridge is
If the panels are not removable for access, carefully locate where it is and cut a neat square hole, preferably to the size of a inspection panel what can be fitted after and will future proof access when needed
This would mean you have low mains pressure/ flow. the taps running raises the flow which will trigger the showers safety sensor which is why this is why it only works when taps running
